camp can't run my code

i don't know what happen about system .i have solving for long time but does not work .i place the code in to lampp/htdocs/folder name/ code name . but it is not work. please help me.

when i run it , i get "Not Found

The requested URL /phpfile/test.php was not found on this server."
i don't know how to do next.
