The difference is, that i could simply take the attachement, throw it into my editors and can check some things.
Posting the content here in a personal message can distort the result. Header information of your file will not be transfered, invisible characters will not be transfered, other letters could be changed, etc.

So, what should i respond? On my environment it is running with your base information and my changes. So either you made something wrong, or your file contains anything which i can't simulate here because the result is distorted, or you didn't told us a needed information, or something like this. I do my best to solve your problem, but slowly i loose my willing to effort much more time into this... Do you want to discuss how you think your problem should be solved by the helpers here or do you want to start to help us to help you?
The first thing you have to do is to revert back the configuration files before the changes you get from O'Reilly. Before this is done i can not provide any more support because of unsupported configuration changes (I can not estimate which effects your changes can have).
